Improved Approval Request management
- Added a comment field for Broker when accepting/rejecting approval request

- Improved UI and filters for exporting

Key Account Manager roles permission changes:
- Removed access from Broker Admin Users page
- Removed access from Broker level Price List
- Added access to editing Subscription Price
- Added ability to edit PO Numbers
- Removed Default Approval Settings cards (only Org-specific settings remain)

Sales Agent role permission changes:
